Defibrillator Threshold Testing
نویسنده
چکیده
Since implantable cardioverter-defibrillators (ICDs) were first introduced, defibrillator threshold (DFT) testing was considered an integral part of the implant procedure. Perform‐ ing DFT testing verifies the ability of the ICD in detecting and aborting the lethal arrhythmia it was designed to treat. The process has evolved with the development of the ICD’s and the expanding indications for their usage. Recent studies even questioned the need for DFT as‐ sessment. Currently this matter is controversial with no firm guidelines. In this chapter we present the basic information on DFT testing, how to manage high DFT, and then discuss the evidence for and against performing the test.

Comparison of outcomes in patients undergoing defibrillation threshold testing at the time of implantable cardioverter-defibrillator implantation versus no defibrillation threshold testing.
BACKGROUND Inability to perform defibrillation threshold (DFT) testing during implantable cardioverter defibrillator (ICD) implantation due to co-morbidities may influence long-term survival. METHODS Retrospective review at The University of Michigan (1999-2004) identified 55 patients undergoing ICD implantation without DFT testing ("No-DFT group"). A randomly selected sample of patients (n =...

BACKGROUND Recent results from the largest multicenter randomized trial (Shockless IMPLant Evaluation [SIMPLE]) on defibrillation threshold (DFT) testing suggest that while shock testing seems safe, it does not reduce the risk of failed shocks or prolong survival. A contemporary systematic review of DFT versus no-DFT testing at the time of implantable cardioverter-defibrillator implantation was...

BACKGROUND Implantable cardioverter-defibrillators and cardiac resynchronization therapy defibrillators have relied on multiple ventricular fibrillation (VF) induction/defibrillation tests at implantation to ensure that the device can reliably sense, detect, and convert VF. BACKGROUND Defibrillation threshold (DFT) testing at the time of implantable cardioverter defibrillator (ICD) insertion is performed routinely. This practice is being reconsidered due to doubts about its ability to improve ICD efficacy and evidence that survival may not be affected by the test. OBJECTIVES To compare the outcome of ICD recipients who underwent DFT testing and those who did not...
متن کاملOrthostatic increase in defibrillation threshold leading to defibrillation failure and prolonged cardiac arrest in a sitting position: Lessons from a patient’s near-fatal experience
Introduction It has previously been reported that patients with an implantable cardioverter-defibrillator (ICD) may have a slightly higher defibrillation threshold (DFT) in an upright position compared to a supine position. However, the significance of this and the potential impact on clinical practice remains unclear. We report a case of defibrillation failure caused by an increase in DFT in t...
